Renal Case #1

case continues:
  • Later that year, this patient is admitted to the hospital for a gram-negative pneumonia.
  • He is started on cefazolin (Ancef, Defzol) 1 gm IV every 6 hours and gentamicin 90 mg IV every 8 hours.
  • His pneumonia starts to clear, but 6 days later, his creatinine has risen from 5 to 10 and his urine now contains numerous granular casts.

What has happened? Were the antibiotics and doses chosen appropriate for the patient? What were other antibiotic choices and how should the doses be adjusted for this patients renal dysfunction?
