Anticoagulation Bridging Before Elective Surgery: What the Guideline Doesn't Cover
A single patient, seven weeks out from a proximal DVT, three weeks from a knee replacement she's counted down to for months. The disagreement isn't about which bridging drug to use — it's about whether the trial everyone is citing ever actually studied someone in her position.
M.T., a 54-year-old woman who has run her own landscape-design business for close to twenty years, spent this spring doing invoices and client calls from a folding chair instead of kneeling in garden beds. Her right knee — worn down, she says, by two decades of exactly the kind of work that built the business — finally reached the point six months ago where her orthopedic surgeon booked a total knee arthroplasty. The date has been circled for months; she has been counting down to it, not dreading it. She has hypertension, diagnosed three years ago and well-controlled on low-dose lisinopril, and longstanding varicose veins in both legs that she'd always treated as a cosmetic nuisance from a career spent on her feet, not a medical one.
Seven weeks ago, with her surgery date already circled on the calendar, she flew home from a landscaping-industry trade conference — a nine-and-a-half-hour flight, most of it spent asleep in a middle seat. She woke the next morning with a swollen, tender left calf; by that evening the swelling had crept to mid-thigh. Ultrasound in the emergency department confirmed an acute proximal (femoropopliteal) deep vein thrombosis. She was started on apixaban 10 mg twice daily for the first week, then stepped down to the standard 5 mg twice-daily maintenance dose she is still taking. Her varicose veins, she learned only then, were never purely cosmetic — chronic venous insufficiency is a real, if modest, predisposing factor, and a long flight on top of it was enough.
Her preoperative clearance visit is where the two problems collided. Her surgeon flagged the recent DVT and apixaban use and referred her for a perioperative anticoagulation consult before clearing the knee replacement to proceed. By the surgery date, she will be roughly ten weeks out from the clot — squarely inside the window every major perioperative guideline treats as high-risk for recurrence, and squarely outside the population the standardized, no-bridging DOAC interruption protocol most of the team already trusts was ever actually tested in.

I'd like to keep the date. She's waited months for this slot, her function has been declining, and a knee replacement isn't the kind of thing you can slide in whenever a schedule opens up. The 2022 CHEST guideline suggests against routine heparin bridging for patients interrupting a DOAC for elective surgery, and that recommendation is written to apply whether the DOAC is being taken for atrial fibrillation or for venous thromboembolism — it isn't AF-specific. Hold the apixaban on the standard schedule, do the case, resume it after.
I recognize a total knee is a high-bleed-risk procedure, not a minor one — I'm not asking to shortcut that part. I'm asking whether the recency of her clot actually changes the interruption plan, or whether that's a separate question from the one the guideline already answered.
It changes the plan, because the guideline you're citing was never tested on someone in her position. PAUSE — the trial the whole no-bridging strategy is built on — enrolled only patients with atrial fibrillation. Nobody with a treatment-dose indication for VTE was in that study at all, let alone someone seven weeks out from the clot. The 2022 CHEST recommendation against DOAC bridging carries a Conditional grade at Very Low certainty of evidence — that grading exists because the evidence underneath it doesn't cover cases like hers.
The same guideline, elsewhere, names a recent VTE within the past three months as exactly the kind of high-risk-for-recurrence patient who might still reasonably be bridged. She qualifies. I'd rather push the surgery back a few weeks and let her get further from the clot than lean on a recommendation that was never built with her in mind. If the date genuinely can't move, I want therapeutic-dose enoxaparin bridging instead of a bare interruption — even knowing bridging carries its own bleeding cost and no proven benefit in exactly this scenario either.
I think the argument over bridging is actually solving the wrong problem. Heparin bridging earned its place with warfarin because warfarin's offset is slow and unpredictable — the heparin covered a gap that could last days. Apixaban doesn't leave a gap like that. Her renal function is preserved, so a standard 48-hour hold before a high-bleed-risk procedure like a TKA already gets her close to a negligible drug level by the time she's on the table, and resuming at 48 to 72 hours post-op closes the other side just as fast. Bridging a DOAC interruption doesn't buy back time the drug's own pharmacokinetics haven't already given up — which is also why the warfarin-era BRIDGE trial found bridging added bleeding without reducing thromboembolism in atrial-fibrillation patients. Though I'll say plainly: that trial didn't study recent-VTE patients either, so it doesn't fully settle this any more than PAUSE does.
So if the real risk here is the multi-day window with no therapeutic drug on board at all — not which drug fills it — the lever that's actually grounded in what we know about VTE recurrence is time, not a bridging agent. Recurrence risk after an unprovoked or transiently-provoked DVT is known to be front-loaded in the early weeks and to decline as the interval since the event grows, which is the whole rationale behind guidelines converging on roughly three months as the point where elective surgery starts looking meaningfully safer to interrupt around. Ten weeks isn't three months. It's closer to it than seven weeks was, and that's the honest best I can offer.
Agreed: if the surgery date holds, apixaban is held starting 48 hours pre-op and resumed 48 to 72 hours post-op once hemostasis is confirmed, with no heparin bridge — the group's pharmacologic reasoning found no gap for bridging to close, and no one round the table disputed that part.
Not agreed, and left as the genuine open question rather than resolved by majority:

Hematology's preference — several more weeks of distance from the clot before any interruption, closer to the 3-month mark the natural-history data actually supports — at the cost of further delaying a surgery she has already waited months for.
Orthopedic Surgery and Hematology left the room with different defaults, and neither pharmacology nor the guideline broke the tie — the guideline's own uncertainty grading is precisely why it couldn't.
