The phrase sitting in the middle of this decision is older than most of the evidence now available to interpret it. "Active liver disease or unexplained persistent transaminase elevations" was the formal contraindication on US statin labels for decades, but current US labeling — pravastatin's included — has narrowed it to acute liver failure or decompensated cirrhosis; the older wording survives in European labeling and in clinical habit. Large cohort data support that narrowing: a 2022 analysis of over 230,000 statin initiators found no meaningfully increased risk of severe liver injury in those who started with an elevated baseline ALT. NAFLD, the single most probable explanation for a picture like D.M.'s, is explicitly not a contraindication in its own right. The word doing the real work in her case is "unexplained" — not because her elevation is inherently more dangerous than an explained one, but because nobody has yet confirmed what it actually is. NAFLD fits her weight and glucose trend, but it is a diagnosis of exclusion, and nothing so far has ruled out viral hepatitis, autoimmune hepatitis, or the rarer metabolic causes the label's caution was arguably written to catch.

Waiting on a low-yield workup has its own cost, and it isn't zero.
I'm not dismissing autoimmune hepatitis as a possibility — I'm saying its low probability doesn't justify delaying a therapy with this much evidence behind it in this population.
I don't think these have to be sequential questions. Pravastatin is hydrophilic and undergoes almost no CYP450-mediated metabolism, unlike the more lipophilic statins — starting it now doesn't require either of you to concede your actual point. The workup keeps moving in parallel exactly as ordered, and if something on it changes the picture, we adjust from a position of already having started, not from having waited and lost the time.
Agreed: start pravastatin 40mg today, continue the already-ordered viral hepatitis serologies, autoimmune panel, and hepatic ultrasound without waiting for the statin decision, and recheck ALT/AST at six weeks alongside those results.
