Reversing Warfarin for Emergency Surgery Around a Mechanical Mitral Valve
A single patient whose emergency surgery is straightforward and whose anticoagulation is not, because the valve keeping her alive can't simply be left unprotected for the sake of a cleaner operating field.
N.P., a 58-year-old woman, has photographed weddings professionally for over twenty years, a career built on being calm under pressure in other people's biggest moments — which made it strange, she said in the emergency department, to suddenly be the one at the center of a crisis instead of documenting someone else's. She arrived with severe right lower quadrant pain, and imaging confirmed acute appendicitis requiring surgery within hours, not days.
Eight years ago, rheumatic mitral stenosis severe enough to threaten her heart function was treated with a mechanical mitral valve, and she has been on warfarin since, with a target INR of 2.5 to 3.5 — higher than the range typically used for a mechanical valve in the aortic position, reflecting the mitral position's greater thrombotic risk. Her INR today is 3.1, therapeutic and, under ordinary circumstances, exactly where it should be. The problem is that ordinary circumstances don't include an appendix that needs to come out this afternoon, and full reversal of her anticoagulation — the standard approach to make surgery as safe as possible — would leave a mechanical valve in a high-risk position with no anticoagulant protection at all during the hours that matter most.
Four-factor PCC reverses warfarin quickly by directly replacing the vitamin K-dependent clotting factors warfarin suppresses, typically within thirty minutes, while IV vitamin K works more slowly but sustains the reversal once the PCC's effect wears off — the two are usually given together precisely because they cover different phases of the correction. What isn't standardized is the target: PCC can be dosed to fully normalize the INR toward 1.0 to 1.2, or to stop short of that; the conventional preoperative target of 1.5 or below is what most surgery is considered safe at, while values above roughly 1.7 retain meaningful anticoagulant effect. How completely to correct her is exactly the decision the team has to make, not a detail that follows automatically once reversal is agreed upon.
How completely to reverse her before the operating room
I'd favor a genuinely partial reversal — bringing her INR down to somewhere around 2.0 rather than into the usual surgical range. Anything at or below 1.5 leaves essentially no anticoagulant effect, so calling that a partial reversal would be wishful. Her valve is mechanical, in the mitral position, and eight years old; that combination carries real thrombotic risk if she goes for even several hours with no anticoagulant effect at all. An appendectomy is generally a manageable bleeding risk even with mild residual anticoagulation on board. A fully unprotected mitral mechanical valve is the bigger danger in this specific window, not the surgical field.
I'd want more complete correction than that. Appendicitis can look straightforward on imaging and still turn out to be a perforation or abscess requiring more extensive dissection once we're actually in there, and I'd rather start from a cleaner hemostatic baseline than discover mid-case that residual anticoagulation is complicating a bigger operation than expected. Anticoagulation can be restarted quickly afterward; a bleeding complication during surgery is harder to walk back.
I think both of those concerns can be answered by the same plan rather than picking one side. Dose the PCC to roughly 1.5 — adequate for the operating field, but short of the full normalization the product is licensed to deliver — low-dose IV vitamin K alongside it, and resume unfractionated heparin as soon as the surgical team confirms hemostasis is secure. Heparin's short half-life means if any bleeding does show up postoperatively, it's the anticoagulant that's easiest to pull back quickly, which buys everyone room to be less than maximally conservative on the reversal target going in.
PCC dosed to an INR target of approximately 1.5 — the conventional threshold for adequate surgical hemostasis, reached without fully normalizing her factor levels — with low-dose IV vitamin K given alongside it; warfarin held. Unfractionated heparin planned to resume within 12 to 24 hours postoperatively, contingent on the surgical team confirming hemostasis.
Heparin bridging starts on schedule, and warfarin resumes once she tolerates oral intake.
The heparin bridge is delayed further, accepting an extended thrombotic-risk window as the lesser immediate danger.
The cardiologist and surgeon never fully agreed on which risk mattered more — thrombosis at her valve or bleeding at the surgical site — and the plan that emerged wasn't a compromise between their positions so much as a specific dosing target designed to avoid needing to fully resolve that disagreement.
