A steroid genuinely built to be safer than its predecessors — and how much caution that improvement actually earns.

The next rung on the treatment ladder, a topical corticosteroid, carries real weight in ophthalmology precisely because ocular steroid exposure has genuine, well-documented risks — cataract formation and steroid-induced glaucoma among them. Loteprednol etabonate, though, was specifically developed as a “soft” steroid: an ester-based structure designed to be rapidly metabolized to an inactive form after exerting its local anti-inflammatory effect. The pooled controlled-trial data behind its labeling put that difference in numbers — clinically significant intraocular pressure elevation of 10mmHg or more in 2% of loteprednol-treated patients against 7% on prednisolone acetate 1% — and Novack and colleagues' long-term analysis found the 0.2% concentration she would actually be prescribed barely separable from vehicle on this measure (0.8% versus 0.7%). That safety improvement is real, but it doesn't reduce the risk to zero, which is the actual disagreement underlying how much caution a genuinely lower-risk option still deserves.
Yuki has no history of glaucoma, elevated intraocular pressure, cataracts, or herpetic eye disease in either eye, and no family history of glaucoma that she's aware of — the specific risk factors that would most directly argue against even a short soft-steroid trial. She wears contact lenses most days but has switched to glasses for the past three weeks specifically because her symptoms made contact lens wear uncomfortable, a change that has itself provided only marginal relief, suggesting contact lens irritation was never a major contributor to her current flare. How much caution a genuinely safer steroid still earns

A short course is a proportionate response to real suffering, not an overreach.
“Softer” doesn't mean risk-free — loteprednol still carries real cataract and glaucoma risk with prolonged or repeated use, just at a lower rate than the older steroids it was designed to improve on.
I'd push her current regimen harder first — higher-frequency mast-cell stabilizer dosing, a cooling compress schedule she actually follows consistently — before reaching for any topical steroid, soft or otherwise.
I don't think this actually has to be steroid-or-not in the abstract. A short, explicitly time-limited course with a scheduled intraocular pressure check converts an open-ended risk into a bounded, monitored one — that's a meaningfully different commitment than either an indefinite steroid course or refusing one outright.
Give the current regimen one more push at maximized dosing this week, and if her symptoms haven't meaningfully improved, start a defined two-week loteprednol course with an IOP check built in before it's extended any further.
