Fourteen Months of Clear Skin, Then a Flare Nobody Can Explain Away
A woman whose atopic dermatitis responded dramatically to dupilumab for over a year suddenly flares with no identifiable trigger, and the question of whether to dose-intensify or switch entirely turns on a specific lab value nobody had thought to check until now.
Bridget O., 33, works as a labor and delivery nurse, a job whose twelve-hour shifts and constant hand-washing had made her severe atopic dermatitis nearly unmanageable before dupilumab, started fourteen months ago, brought her EASI score from 32 to 2 within ten weeks and kept her there — clear enough skin, for the first time in years, that she stopped wearing long sleeves under her scrubs. Three weeks ago, with no identifiable trigger — no new soap, no missed doses, no illness, no change in her shift pattern — her EASI climbed back to 19, a flare severity she hadn't seen since before starting the drug.
Secondary loss of response after a year or more of excellent control is a recognized, if uncommon, phenomenon across biologic therapies generally, and the mechanism matters enormously for what fixes it. A large pooled immunogenicity analysis of dupilumab by Kamal and colleagues, across seven phase 3 trials and two long-term open-label extension studies in atopic dermatitis, found treatment-emergent anti-drug antibodies (ADA) in up to 8.6% of adult patients — but the antibody presence itself barely moved outcomes; it was specifically the smaller subset with high-titer responses (above 10,000, on the assay's scale) that showed meaningfully lower serum dupilumab concentrations and loss of efficacy, independent of whether the antibodies were neutralizing. Nobody had checked Bridget's antibody status before this visit, because nobody had needed to — fourteen months of clean response gave no reason to look.
Bridget has spent the last three weeks doing what she describes as detective work on her own life, cataloguing every soap, lotion, and laundry product in her apartment for a change she might have missed, mostly, she admits, because the alternative explanation — that a drug that worked perfectly for over a year could simply stop working for a reason nobody chose or caused — felt harder to accept than a hidden trigger would have. Returning to work in visibly flared skin, after a year of colleagues and patients not noticing her eczema at all, has been its own quiet blow. The hidden cost of her search for a trigger is that the one explanation it cannot turn up is the one that would change the treatment: an antibody titer is not something a person finds by auditing their own laundry detergent, and every week spent looking is a week the untested mechanism goes on deciding the outcome.
What her titer knows that a guess doesn't
My first instinct is to shorten her dosing interval — dose intensification is the standard first move for secondary loss of response across most biologic classes, and it's something I can act on today rather than waiting on a specialized antibody assay while she continues to flare through her shifts.
I'd check her anti-drug antibody titer and trough dupilumab level before intensifying anything, because the two possible causes of her flare call for genuinely different fixes. The largest immunogenicity dataset we have — a pooled analysis across seven phase 3 trials and two open-label extension studies, published by Kamal and colleagues in Frontiers in Immunology — found that treatment-emergent antibodies occurred in up to 8.6% of adult patients but barely affected outcomes at low or moderate titer. It was specifically the high-titer responses, under 1% of patients, that were associated with meaningfully reduced serum drug levels and loss of efficacy. If Bridget has a high-titer antibody response, dose-intensifying might do very little, since more drug going in doesn't help if it's being neutralized as fast as it arrives.
I don't think dose intensification is a wrong instinct in general — it's the right move for genuine subtherapeutic exposure — I just don't think we know yet which problem we're actually looking at, and the titer test answers that directly rather than leaving us to guess.
If the titer does come back high, I'd lean toward switching her to tralokinumab rather than further intensifying dupilumab. Tralokinumab targets IL-13 directly rather than blocking the shared IL-4 receptor alpha subunit dupilumab acts on — a structurally distinct biologic that a dupilumab-specific neutralizing antibody wouldn't be expected to cross-react with. That sidesteps the antibody problem entirely rather than trying to out-dose it, though it does mean starting a second biologic's own onset curve and safety profile from scratch rather than continuing a drug she's already fourteen months into.
Bridget's titer returned high — above the assay's 10,000 threshold — with a correspondingly low trough dupilumab level, matching the pattern the pooled immunogenicity data associate with genuine loss of efficacy. All three agreed this pointed toward tralokinumab rather than dose intensification, since more dupilumab was unlikely to overcome a high-titer neutralizing response already clearing the drug faster than it could act.
The switch to tralokinumab was made with an explicit plan to recheck her EASI at eight and sixteen weeks, the drug's typical onset window; all three clinicians noted this case as a useful, concrete example of why checking the mechanism before choosing the fix mattered here — an empiric dose increase, tried first on the more common conventional instinct, would likely have cost her several more weeks of flare before failing for a reason nobody had yet confirmed.
