Resuming the Beta-Blocker Right Off Bypass, Before the Conduction System's Status Is Known

A 76-year-old woman with a heavily calcified aortic valve is off bypass after AVR, and her home metoprolol is due. Resuming it protects against the arrhythmia her abrupt withdrawal would otherwise risk — and could also mask or worsen a conduction injury nobody has confirmed is absent yet.

Presentation

Beatrice N., a 76-year-old woman, has led her church's Wednesday bible study for over twenty years and has taken metoprolol for hypertension for nearly as long, well controlled throughout aside from the severe aortic stenosis found last year on a routine physical exam that picked up a new murmur she had not noticed any symptoms from yet. Her home dose is metoprolol 50mg twice daily, held this morning per routine protocol — which means that by the time the question of resuming it arises, she will have gone the better part of a day without a drug her cardiovascular system has been calibrated around for two decades.

Her valve, when her surgeon finally had it in view today, turned out more heavily calcified than the preoperative echocardiogram had fully conveyed — dense, nodular calcium extending down toward the membranous septum, close enough to where the atrioventricular node and proximal conduction system run that her surgeon flagged the anatomy directly to the team before closing. She is off bypass now, hemodynamically stable with a MAP of 72 and no vasopressor requirement, in a junctional rhythm at 58 that her surgeon is watching closely rather than dismissing as an unremarkable post-bypass finding. Given the calcification he described, that rhythm has two very different explanations: it could resolve on its own as bypass effects wear off, or it could be the first visible sign of conduction injury from debriding calcium that close to the node. I'd resume her metoprolol on schedule, as soon as her pressure allows it. Abrupt withdrawal produces real rebound sympathetic activity — reflex tachycardia and hypertension — and that's a well-documented driver of postoperative atrial fibrillation, which is exactly what perioperative beta-blockade is supposed to be protecting her from in the first place.

I'd hold off for now, specifically because of what I saw when I had her valve open. That calcification extended close to the membranous septum — close enough to the AV node that I flagged it to the room before we even closed. She's junctional right now, and I don't yet know if that's going to resolve on its own or if it's an early sign of conduction injury.

Giving a negatively chronotropic drug before we know which one it is risks masking the distinction, or actively pushing a borderline conduction system further in the wrong direction. I'd rather watch her rhythm for the next hour with nothing added to it than resume metoprolol reflexively on a fixed schedule.

That's a real, specific anatomic concern, not a generic caution, and I don't think the withdrawal-rebound argument is strong enough to override it — you're right that resuming metoprolol into a rhythm we haven't characterized yet could make it genuinely harder to tell what's happening, or make it worse.

I'll hold it for now. If her rhythm converts back to sinus and stays stable over the next hour, I'd want to resume it before she leaves the OR rather than let a full day pass — the withdrawal risk is still real, just not more urgent than confirming her conduction system first.
