Lowering the Pressure Just Enough to Qualify for Reperfusion

A retired bus driver's pressure is too high to legally give him the clot-busting drug that could reverse his stroke. Getting him under the threshold without overshooting it is its own decision, made against a clock.

Presentation

W.P., a 72-year-old retired city bus driver, was watching a ballgame at his son's house when his speech suddenly stopped making sense and his right arm dropped into his lap. He arrived ninety minutes past last known well with a moderate M2 occlusion and a genuinely salvageable penumbra on perfusion imaging — a real thrombolysis and thrombectomy candidate on the clock, except for one number standing between him and treatment: a blood pressure of 210/118, well above the 185/110 threshold tPA eligibility requires. His own account, delivered in the fragments his aphasia allows, is that he's missed blood-pressure doses “more than I should” for months, a history that explains today's number without changing what has to happen to it in the next few minutes.

The instinct to lower a pressure this high quickly is understandable, but INTERACT4, the 2024 trial testing prehospital ultra-early blood pressure lowering before a stroke's type was even confirmed, found a real asymmetry worth naming directly: aggressive early lowering improved outcomes in the patients who turned out to have hemorrhagic stroke (common odds ratio 0.75) and worsened them in the patients with cerebral ischemia (1.30). That split is reported by stroke type and nothing finer — the trial did not separate its ischemic patients by whether they went on to reperfusion, so it contains no subgroup that describes him specifically, and its own investigators drew the uncomfortable lesson rather than the reassuring one, saying the finding should make clinicians more cautious about dropping pressure ahead of thrombolysis. What obliges the team to lower his pressure is not INTERACT4 but the eligibility threshold itself. The target here is not aggressive normalization — it is the narrowest, most controlled reduction that clears the 185/110 threshold without dropping mean arterial pressure enough to compromise the penumbra his perfusion imaging still shows is worth protecting.

Speed matters, but so does not overshooting — INTERACT4 found early lowering worsened outcomes across its ischemic patients as a whole, and it never carved out the ones who went on to reperfusion — so I can't tell you he sits in some protected slice of it. The harm signal is the part that applies, and the only reason we're touching his pressure at all is the 185/110 threshold. A labetalol bolus is genuinely harder to fine-tune once given; if it drops him further than we intend, we can't titrate it back the way we can stop or slow an infusion. Nicardipine gets us the same threshold with a margin we can actually control in real time.

Agreed on nicardipine, and one addition: don't layer labetalol on top of it if the first response feels slow — stacking two agents with different offset profiles right when fine control is the entire point would make an overshoot harder to reverse, not easier. Titrate nicardipine alone to the narrowest reduction that clears 185/110, and treat 'clears the threshold' as the actual target, not a floor to push meaningfully past.
