Grace L., a 46-year-old paralegal, has taken 100 micrograms of levothyroxine daily for nine years with a TSH that has stayed between 1.5 and 2.3 μIU/mL at every check for the past four. Two weeks ago her pharmacy switched manufacturers on a routine refill, a change she only noticed because the pill looked a slightly different shade of yellow, and within days she began noticing what she describes as jitteriness at her desk and a racing feeling at night that has twice kept her awake. She called her doctor's office specifically to ask whether the new pills could be "weaker or stronger," a question she'd never had reason to ask in nine years on the same medication. She denies any other new medications, supplements, or illness, and her weight and appetite are unchanged.

Levothyroxine's narrow therapeutic index is exactly why this question can't be waved off, and exactly why it also can't be assumed true without checking.
