I think this doesn't have to be resolved as argatroban-versus-fondaparinux on an uncertain liver reading either way. Bivalirudin clears through both enzymatic degradation and renal elimination — it doesn't depend on either organ system being the sole pathway, which removes the entire question you're both debating rather than picking a side of it. Its HIT evidence base is smaller and drawn more from cardiac and PCI populations than orthopedic ones like him, and that's a real, named tradeoff, not a free win.

Given that tradeoff is honest and his platelet count needs treatment now, not after further hepatic workup, I'd start bivalirudin today with aPTT-based titration and revisit if his response doesn't track the way it should.
