Starting a Complement Inhibitor Against a Two-Week Vaccination Window
A new PNH diagnosis with an active clot forces a decision about starting a complement inhibitor before the labeled two-week meningococcal vaccination window has passed.
Marcus T., 34, has spent the past eleven years running concrete crews, work that leaves him wrecked most evenings even on a good week, and lately every week has felt like a bad one. He is getting married in five weeks and had chalked up the fatigue and the dark morning urine to wedding-planning stress until his fiancee insisted he see someone. Flow cytometry confirmed paroxysmal nocturnal hemoglobinuria with a large granulocyte clone, and this admission — prompted by a swollen, tender left calf that turned out on ultrasound to be a new deep vein thrombosis — is his first real look at what the disease can actually do. He has never had a clot before, doesn't smoke, and has no other cardiovascular risk factors; the thrombosis is the PNH, not an unrelated event layered on top of it.
His LDH is 1,850 U/L against a laboratory ceiling of 250, better than seven times normal and consistent with the ongoing complement-mediated intravascular hemolysis driving both his fatigue and, mechanistically, his clot — free hemoglobin scavenges nitric oxide and activates platelets in a way that makes PNH thrombosis disproportionately dangerous, historically accounting for the largest share of PNH deaths in the era before complement inhibition existed. Ravulizumab, a terminal complement (C5) inhibitor, is the therapy that actually addresses the mechanism rather than just anticoagulating the clot he already has, and its longer half-life relative to its predecessor eculizumab means once-every-eight-weeks maintenance dosing rather than biweekly infusions — a real practical advantage for a man whose work doesn't come with easy time off for recurring clinic visits, though it changes nothing about the vaccination-timing question in front of the team tonight.
The complication is what blocking C5 also does: it disables the membrane attack complex his body would otherwise use against Neisseria meningitidis, and the FDA's own labeling — enforced through a formal REMS program — calls for meningococcal vaccination completed at least two weeks before the first dose, precisely the two weeks his hematology team doesn't think his clot burden can wait for.
Before the first infusion, racing a two-week label
Start ravulizumab today, not in two weeks. Thrombosis was the single largest cause of death in the pre-complement-inhibitor era. Hillmen and colleagues' 1995 Hammersmith natural-history series — eighty patients followed on supportive care alone, median survival ten years from diagnosis — found thromboembolism contributing to death in twenty-eight of the forty-eight patients whose cause of death was known, and it remains the leading cause of death in undertreated disease today. He already has a clot; his LDH tells us the hemolysis driving it hasn't slowed at all. Waiting two weeks for vaccine-conferred immunity to develop, in a patient whose disease has already crossed from biochemical abnormality into a life-threatening thrombotic event, asks the wrong question first.
Nobody in this room is going to make him wait two weeks with a propagating clot, so let's set that option aside — I'm not arguing for the full delay pathway. The label itself doesn't require it; it offers a bridge specifically for this situation: vaccinate today, start the infusion today, and cover the immunity gap with antibiotic prophylaxis until protection develops.
What I'd push back on is treating two weeks of prophylaxis as the end of the conversation. McNamara and colleagues, reporting in MMWR in 2017, described a cluster of breakthrough meningococcal disease in eculizumab recipients who had been appropriately vaccinated — vaccination reduces but doesn't eliminate risk on this drug class, because the vaccine's own protective mechanism depends partly on the same terminal complement pathway the drug is blocking. That's why several centers now extend prophylaxis for the duration of therapy rather than stopping it once vaccine immunity would ordinarily be presumed adequate.
Practically, that means penicillin VK twice daily, or a macrolide if he's allergic, started alongside today's first dose and continued rather than time-limited to fourteen days, given what was just described. He needs the meningococcal conjugate and serogroup B vaccines both, ideally at two different visits per standard practice but today if that's what urgency requires.
He also needs to leave here understanding that any fever on this drug is a same-day emergency regardless of how long he's been vaccinated or how well the last infusion went — that's the actual content of the REMS counseling requirement, not just a signature on a form.
Agreed same day: ravulizumab initiated today rather than delayed, both meningococcal vaccines given today rather than staggered, and penicillin VK prophylaxis started alongside the first infusion.
Not agreed as settled guideline, only as this team's own choice: how long the prophylaxis continues. The infectious disease physician's preference — for the duration of complement-inhibitor therapy, given the real breakthrough-disease data even in vaccinated patients — isn't yet the universal standard elsewhere, and the hematologist noted most PNH centers she's worked with still stop at the labeled two weeks. They chose the longer course for him specifically, not as a claim about what every future patient should get.
