Severe Nephrotic Syndrome and Recent Surgery: When Does the VTE Risk Justify Anticoagulation

A single patient with albumin low enough, and an added immobility risk, to make prophylactic anticoagulation a real question rather than a formality. The disagreement is whether two independent risk factors should be treated as one open-ended decision or two separately timed ones.

Presentation

George P. had his right knee replaced five weeks ago after years of putting it off, and was three weeks into what he assumed was a routine recovery — walker, physical therapy twice a week, slower than he'd like but expected — when swelling that seemed disproportionate to a healing knee led his surgeon to check labs that turned out to have nothing to do with the knee at all. Albumin of 1.8, protein in his urine at 9 grams a day: severe nephrotic syndrome, confirmed on biopsy as membranous nephropathy the following week. He is seventy, has no personal or family history of blood clots, and is still using a walker for most of his ambulation while his knee continues healing.

The albumin number alone already crosses into the range most clinicians use as a rough trigger for considering prophylactic anticoagulation — nephrotic-range proteinuria drives real losses of antithrombin III and protein S in the urine, while fibrinogen rises, and membranous nephropathy specifically carries the highest VTE risk of any nephrotic cause. Lionaki and colleagues put a number on the albumin part of that in 2012: each 1 g/dL fall roughly doubled VTE risk, with 2.8 g/dL the threshold below which risk was greatest — his 1.8 sits a full point under it. Lee and colleagues then built those variables into a formal decision-analysis model weighing thrombotic against bleeding risk, which is what turns a threshold into an actual individualized recommendation rather than a gut call. What makes his case more than an arithmetic exercise is that he has two real risk factors sitting on top of each other with genuinely different expected durations: the hypoalbuminemia, which should improve once his membranous nephropathy is treated, on a timeline measured in months; and the post-surgical immobility, which should resolve as his knee recovery finishes, on a timeline measured in weeks. Treating those as one open-ended anticoagulation decision, rather than two separately timed ones, is the kind of default that's easy to reach for and not obviously the right one. His walker, sitting by the bed where the nephrology team examines him each morning, is a small physical reminder of exactly which of his two risk factors is acting on him right now — the albumin will keep falling for weeks yet even after treatment starts working, while the immobility is already, today, the more acute contributor to his risk.

Waiting for the albumin to recover treats his immobility risk as though it doesn't exist independently — it does, right now, regardless of what his albumin does over the next two months.

I think you're both right about different halves of this. The albumin-driven risk is real and will take months to resolve — holding anticoagulation and just waiting for that ignores the immobility risk sitting on top of it right now. But committing to open-ended anticoagulation pegged to the albumin number also overstates how long the more acute part of his risk — the immobility — actually lasts.

I'd bridge with prophylactic enoxaparin specifically through his post-surgical recovery window, reassessed at four weeks when his mobility should be substantially improved, independent of where his albumin stands at that point. If his albumin is still low once the immobility risk has resolved, that's a separate decision to make then — not the same clock as this one.
