TTP With Neurologic Symptoms: Does Caplacizumab's Bleeding Tradeoff Still Favor Adding It

A single patient, hours into emergent plasma exchange for TTP with neurologic involvement. The disagreement isn't whether caplacizumab works — a real trial already showed that — it's whether its named bleeding cost is worth paying before the underlying disease treatment has had time to act.

Presentation

Naomi F. was previously healthy — no medical history worth mentioning until yesterday, when a headache that wouldn't quit turned into brief confusion her husband noticed before she did, confusion that had resolved by the time the emergency department drew labs but left everyone in the room unwilling to write it off as nothing. Platelets came back at 9,000. ADAMTS13 activity, sent urgently, came back severely deficient at under 10% — acute TTP, confirmed rather than presumed, and plasma exchange started within hours alongside glucocorticoids while the team worked through what else belonged in the initial regimen.

The transient confusion is the detail actually driving this conversation, more than the platelet count alone would. HERCULES randomized caplacizumab — an anti-von Willebrand factor nanobody that blocks the platelet-adhesion step driving TTP's microthrombi directly — added to standard PLEX and immunosuppression, and found real, meaningful benefits: faster platelet count normalization and a reduced composite of TTP-related death, recurrence, and major thromboembolic events during the study period. That's not a marginal result. But the trial also found a real, named tradeoff — increased bleeding events — and a signal in some analyses that exacerbations were more common after caplacizumab was stopped, which complicates a simple 'faster is always better' reading. Her neurologic symptoms, even though they've already resolved, are the kind of detail that argues her disease sits toward the more severe, higher-risk end of TTP's spectrum, which is where the case for accepting caplacizumab's bleeding cost in exchange for faster stabilization gets strongest — and exactly where getting the sequencing with PLEX right, not just the yes/no decision, starts to matter as much as the drug choice itself. Her husband has already asked, twice, whether the confusion could come back — a fair question the team can't fully answer yet, and one that sits underneath the caplacizumab decision as much as any lab value does, since the honest reply depends on how quickly and how completely her platelet count and her ADAMTS13 activity both recover, not just one or the other.

HERCULES showed caplacizumab gets platelets back to normal faster and reduces the composite of death, recurrence, and thromboembolic events. I'd add it now rather than wait to see how she does on PLEX and steroids alone.

Hematologist (hold, reserve for refractory disease) Response

I take the neurologic history seriously, but it's already resolved, and HERCULES also found a real increase in bleeding events with caplacizumab — that's not a footnote, it's a documented tradeoff. She has no active bleeding and no recent surgery. I'd start PLEX and immunosuppression, give ADAMTS13-directed therapy time to work, and reserve caplacizumab for exacerbation or clear non-response.

Waiting for exacerbation before adding caplacizumab means accepting the slower platelet recovery HERCULES specifically showed we can avoid — in a patient who's already had one neurologic event, that's not obviously the more conservative choice.

Clinical Pharmacologist (add with adjusted PLEX taper) Final

I'd add caplacizumab given her neurologic involvement — I think that argument is right. But I want to flag something the bare yes/no framing misses: caplacizumab can normalize her platelet count on its own, independent of whether her ADAMTS13-directed immunosuppression has actually achieved remission yet. If we treat a normal platelet count as our usual signal to taper PLEX, we could stop plasma exchange before the actual disease process — the antibody against ADAMTS13 — is under control.

So: add caplacizumab now, but plan a slower PLEX taper than we'd normally use, gated on ADAMTS13 activity recovery rather than platelet count alone. That gets us the faster stabilization HERCULES showed without letting caplacizumab's own effect mislead us about whether the underlying disease is actually resolving.

The reserve-for-refractory-disease position wasn't rejected as wrong in principle; both hematologists agreed it would have been reasonable absent her neurologic event, which was what specifically tipped the decision toward adding caplacizumab now rather than waiting.
