I take the contraindications seriously, but I don't think 'the alternatives are limited' automatically means an opioid-class drug is the right next step. The 2019 Cochrane review of tramadol in osteoarthritis is the honest summary here, and it is harsher than the drug's reputation: moderate-quality evidence that tramadol has no important benefit on mean pain or function against placebo, with only slightly more patients reporting an improvement they'd call meaningful. What it does reliably produce is withdrawal for adverse events — a relative risk of 2.64, about a twelve-percentage-point absolute increase over placebo. That trade reads differently at seventy-two — falls, sedation and dependence potential in exactly the age group he's in.

If we've genuinely run out of medical options for a 72-year-old, that's a signal to push the cardiology timeline, not necessarily to start a chronic-use drug for a problem that already has a scheduled fix.

Start it now, but frame it explicitly as a bridge with a defined endpoint — his surgical date, or a re-evaluation point if the cardiac workup runs long — rather than an open prescription.
