Renal Case #1

case continues:
  • His gout flares and he is seen in a local emergency department.
  • He is given colchicine and ibuprofen.
  • Four days later he calls complaining of shortness of breath, edema, and diarrhea.
  • When you see the patient, his BP is 190/110; he has rales throughout his chest and has a large amount of peripheral edema.
  • His serum creatinine is now 9.

What has happened? What could have been done to avoid these problems?
