New AF, Old HIT: Why Starting Warfarin Isn't as Simple as Just Avoiding Heparin
A remote heparin-induced thrombocytopenia history and a brand-new reason to need long-term anticoagulation. The complication isn't which drug treats her AF — it's what starting each option actually requires from a patient who can never touch heparin again.
S.W., a 66-year-old woman, spent twenty-eight years as a public librarian before retiring, and still volunteers cataloging donated books for the library's annual sale. Five years ago, after a knee replacement, she developed heparin-induced thrombocytopenia — confirmed by a positive PF4 antibody test and a falling platelet count that recovered once all heparin was stopped and she was switched to argatroban, then bridged to warfarin for a few months before her orthopedic team cleared her to stop anticoagulation entirely. She has avoided heparin in every hospital encounter since. She also carries longstanding hypertension and type 2 diabetes, both stable on treatment. Today she has a new diagnosis: paroxysmal AF, found incidentally on a routine ECG, with a CHA₂DS₂-VASc score of 4 — one point each for hypertension, diabetes, age 65 to 74, and female sex — placing her clearly inside the range where anticoagulation is recommended rather than merely reasonable. She has no mechanical heart valve, normal renal function, and no other reason a DOAC would be contraindicated for her specifically — but her HIT history changes what starting either major anticoagulant option actually involves.
A DOAC requires no bridge of any kind — full anticoagulant effect from the first dose, no heparin exposure at any point in the process. Warfarin is different, and for a reason worth stating precisely: it suppresses the vitamin K-dependent factors at different speeds. Protein C and protein S — the body's own natural anticoagulants — have short half-lives and fall within hours, while factor II takes days to clear. Starting warfarin alone therefore removes anticoagulant capacity before it removes procoagulant capacity, creating a transient hypercoagulable window that can itself cause dangerous clotting unless it is covered by a fast-acting parenteral anticoagulant. In a patient with a HIT history that risk has a specific name and a specific shape: venous limb gangrene. For most patients, that bridge is heparin or low-molecular-weight heparin — options permanently closed to S.W. For her, a fresh warfarin start would require a non-heparin parenteral agent, typically argatroban or fondaparinux, overlapped for at least five days before warfarin alone is enough. The field's own guidance has shifted here too: the 2018 ASH guideline now lists DOACs as an acceptable option even during active HIT itself, a meaningful move away from the parenteral-then-warfarin sequence that was standard when S.W. was first diagnosed.

This isn't a claim that warfarin is unsafe for her — it's that the DOAC path is simply cleaner given her specific history.
Warfarin has decades of real-world outcome data behind it, and the bridging requirement is a real but bounded cost — five days of a non-heparin parenteral agent, not an ongoing burden. I wouldn't rule it out by default just because it requires more coordination to start.
