SSRI + NSAID/Anticoagulant Bleed Risk
She needs an antidepressant, a chronic NSAID for a bad knee, and an anticoagulant for her heart rhythm — three separately reasonable medications that stack on the exact same bleeding risk.
I.R., a 68-year-old retired librarian, has three concurrent, independently well-justified medical needs that have converged on the same visit. She was diagnosed with atrial fibrillation eighteen months ago and has been on apixaban since. Her CHA2DS2-VASc is 3 — one point for age 65 to 74, one for hypertension, one for female sex — which clears the threshold at which anticoagulation is recommended rather than merely considered, and remains a 2 on the sex-independent CHA2DS2-VA score that recent guidelines favor. She has significant osteoarthritis in her right knee, managed for the past year with daily ibuprofen 600mg, which she describes as the only thing that lets her keep up with her grandchildren without significant pain. And she now has a new moderate depressive episode, her first, with a PHQ-9 of 17, following her husband's recent diagnosis with a serious illness.
Each of these three treatments is individually reasonable and, taken alone, not unusual. The problem is that they don't operate independently of each other on bleeding risk. SSRIs deplete platelet serotonin through the same serotonin transporter they act on in the brain, and because platelets can't synthesize new serotonin once it's depleted, this effect persists for the platelet's full lifespan — a real, independent contributor to bleeding risk, most notably gastrointestinal, that exists even without any other bleeding risk factor present.
Stacked on top of her existing apixaban, an SSRI would combine that platelet effect with direct factor Xa inhibition — a combination already associated with meaningfully elevated GI bleeding risk in observational data. Her daily ibuprofen adds a third, mechanistically distinct contributor: direct COX-mediated injury to the gastric mucosa, on top of whatever bleeding risk the other two drugs already carry. No single piece of this regimen is unreasonable in isolation, and stopping any one of the three has its own real cost — undertreating her stroke risk, undertreating her knee pain, or undertreating her depression. The team's task is finding a combination that treats all three conditions while reducing the compounded bleeding risk as much as the constraints allow.
At the medication-reconciliation visit
The platelet mechanism is worth being specific about rather than treating this as a vague 'SSRIs and bleeding' caution — platelets take up serotonin via SERT and release it during aggregation, and once an SSRI depletes that store, platelets have no way to resynthesize it for the rest of their roughly ten-day lifespan. That's a real, mechanistically distinct contributor to bleeding risk, independent of and additive to whatever her apixaban and ibuprofen are already doing.
I don't have room to negotiate on the apixaban — at a CHA2DS2-VASc of 3 she is above the threshold where anticoagulation is recommended outright, not in the discretionary band, and stopping or reducing it to make room for an antidepressant would be trading a well-quantified stroke-prevention benefit for an unquantified bleeding-risk reduction. Her ibuprofen is also working against the amlodipine, which is a second reason to move it. Whatever changes happen here need to happen on the other two drugs, not this one.
Then let's reduce the stack from the two sides that can actually move. Switching her ibuprofen to a topical NSAID — which guidelines actually favor over acetaminophen for knee osteoarthritis, and which delivers a fraction of the systemic exposure — removes essentially all of the COX-mediated mucosal injury while keeping an anti-inflammatory at the joint, with acetaminophen available alongside it. And mirtazapine treats her depression without SSRI-class SERT-mediated platelet effects, since its mechanism doesn't run through serotonin reuptake the same way. I'd add a PPI as well given she's still on apixaban regardless of what we do with the other two.
Mirtazapine 15mg was started for her depression, oral ibuprofen was switched to a topical NSAID with acetaminophen available alongside, and omeprazole was added given her ongoing apixaban — apixaban itself was continued unchanged, per the cardiologist's assessment of her stroke risk.
Nearly all of the systemic NSAID contribution is removed from her regimen, leaving apixaban and mirtazapine as the dominant remaining considerations, with PPI coverage in place.
Intra-articular corticosteroid injection is the next step rather than resuming an oral NSAID, keeping systemic COX inhibition — and its bleeding-risk contribution — out of the regimen as much as possible.
The team's reasoning was explicit that no single medication was the problem — the risk came from how three independently reasonable treatments stacked on the same bleeding-risk pathway, and the fix was reducing that stack wherever a substitution existed, not abandoning any of her three underlying diagnoses.
